The Mental Health and Wellness Checklist was developed to provide individuals the ability to "check-in" with themselves in areas that may be neglected. You may not necessarily need full-blown therapy, but you are interested in fine tuning your mental health and overall wellness.
If you provide your email, you will receive a free summary email of your results. Don't worry, information is only accessible by me. If you do not provide an email, you can download your results in a PDF file once you submit your answers.
I offer 30-45 minute mental wellness check-ups via telehealth. Just enough time for us to discuss areas for improvement, facilitate a plan, and schedule our next checkup, typically every 3 months (quarterly).
